Diabetic Days
Beginning December 1, First Choice
Community Health Centers (FCCHC) is happy to announce the
unveiling of Diabetic Days, a new program aimed at meeting the
special needs of our diabetic patients. We will continue
to see anyone during these times as well as see our diabetic
patients at other times if it is more convenient for them.

Diabetic Days will
feature special opportunities including: referrals to
specialty care, patient education, nutrition counseling, meter
