SLIDING FEE PROGRAM REQUIREMENTS
As Federally Funded Health Centers, First Choice Community Health Centers receive a grant from the Bureau of Primary Health Care (BPHC) to provide medical and dental services to the uninsured and under-insured residents of Harnett County and surrounding areas. Our Sliding Fee Program allows for reduced fees for services provided in our clinics for all individuals and families who qualify. If you apply and qualify at one location, you will receive the discount at all of our clinic locations.
BILLING INFORMATION
First Choice Community Health Center places a strong emphasis on accurate documentation of patient and insurance information during registration. They accept most insurance plans and expect patients to be aware of their co-payment, co-insurance, and deductible responsibilities, encouraging them to contact their insurance carrier beforehand. The Billing Office diligently handles insurance claims, correcting any issues, and filing with secondary insurance when necessary. Patients are billed for any remaining balances after primary insurance payments are processed and receive monthly statements. For those in need, a Sliding Fee Discount Program offers financial assistance based on income and family size, with nominal fees for individuals and families at or below 100% of the Federal poverty guidelines and sliding discounts for those between 100% and 200% of poverty.
