PATIENT COVID-19 VACCINATION AND TESTING

By appointment only
To protect the health and safety of our staff and patients, walk-ins cannot be accommodated at this time
Please have your ID and insurance card (if applicable) at your vaccination/test appointment
Upon arrival proceed to the front of the building for screening
A provider will meet you at the front entrance and direct you to the COVID-19 clinic

FCCHC will be adhering to the current NCDHHS guidelines for vaccination by group and will be provided for anyone in the currently active groups as defined by the NCDHHS. For more information or to see if you qualify please visit

Registration Requirements

You will be required to complete registration forms that ask several important questions complying with FCCHC standards and regulatory reporting procedures

A registration form will be required for EACH person being vaccinated.

Insurance Information

You DO NOT need insurance to get a vaccine. The is free. Insurance information does not affect eligibility. If you have insurance it will be billed, BUT YOU WILL NOT BE CHARGED!

Call 911 if you have a medical emergency: Notify the operator that you have, or think you might have COVID-19. If possible, put on a cloth face covering before medical help arrives.
